Attention is invited to the outlines of the human skeleton and figure upon the back of this certificate, and they should be used whenever it is possible to indicate precisely the location of a disease or injury, the entrance and exit of a missile, an amputation, &c. The absence of a member from a session of a board and the reason therefor, if known, and the name of the absentee, must be indorsed upon each certificate.

We hereby certify that in compliance with the requirements of the law we have carefully examined this applicant who states that he is suffering from the following disability, incurred in the service, viz: Rheumatism, wound of clavicle
If a pensioner, state the amount, if known, of the whole line. and that he receives a pension of dollars per month.
He makes the following statement upon which he bases his claim for Orig. [Original, Increase, or Restoration.] pension: was struck by a shell (1864) & bullet (left) was radiated lower in body, contracted rheumatism in '64 & got no worse
can't work, on account of arms & shoulders
Upon examination we find the following objective conditions: Pulse rate, 110 respiration, 24 temperature 99 1/2; height, 5 feet 6 inches; weight, 148 pounds; age, 46 years. He has rheumatism in left leg. Left shoulder lower than joint at top & painful, & positive disability are limited on account of can't raise his left arm horizontal with the body, & has rheumatism to some degree. Pains in region of heart that are very severe, made above shoulder & no permanent relief. Spine rigid, shoulders very tender, space that normal width active, to some degree are directed at that is perceptible in left operative spine indicating hypertrophy & dilation of heart. Not unreliable.
His left clavicle has been fractured to 2 pieces, 2 inches long removed, the union of fracture to sternum has prevented raising left shoulder to elbow archy over his arm & raised at an angle of 30 degrees at the elbow can't raise right arm to the horizontal. If any form limb, with his shoulder all the time, there is no union with the right clavicle. The arm is stable as long as except there, he is sound & healthy.
He is, in our opinion, entitled to a 4/18 rating for the disability caused by Rheumatism 4/18 for that caused by heart disease and 16/18 for that caused by wound of clavicle & anchylosis of shoulder & sternum.
